By submitting this form, the attendee and their parent or guardian understand and accept the following terms and conditions of the Art in Makeup Experience Camp herein provided:

1. Camp will be on a first come first served basis.
2. The payment and submission of this form enrolls the attendee to the selected camp session.
3. Withdrawal of enrollment prior the start of the camp shall deduct 25% from the full amount provided to be returned to the attendee or guardian.
4. Should the attendee withdraw or not show up anytime within the period of the enrolled camp no refund will be issued.
5. No food or drink is provided in this camp experience, attendees may bring snacks and drinks to camp.
6. International Institute of Cosmetology nor its staff is responsible for lost or stolen items. Attendees should be mindful of their belongings.
7. The International Institute of Cosmetology reserves the right to cancel part of/or the entire program.
8. The camp’s staff may take photos or videos while in the camp’s activities and may use such photos or videos for promotional purposes. However, attendee or parent/guardian does not wish images/video to be used, Please inform us anytime prior to start of camp.
9. The attendee and/or parent or guardian authorizes the International Institute of Cosmetology to seek medical treatment in the event of an accident or sickness. The camp’s staff will notify the emergency contact of the emergency as soon as possible.
15. International Institute of Cosmetology reserves the right to cancel, amend or alter activities accordingly in the event of unanticipated situations.
16. Privacy Disclosure: I, attendee/parent/guardian hereby consent to receive autodialed and/or pre-recorded telemarketing calls , emails and text messages (standard text message charges may apply) from or on behalf of International Institute of Cosmetology at the telephone number and/email address provided. Attendee/parent/guardian understands that consent is not a condition of purchase. attendee/parent/guardian also gives the International Institute of Cosmetology permission to store their data for marketing, enrollment and contacting purposes.
